Children with Disabilities Team (CWD)

Social Care / Early Help


Who are CWD?

The Children with Disabilities Team (CWD) team are based at Jubilee Gardens, Droylsden.  The team comprises of Social Workers, Children with Disabilities (CWD) Intervention Worker, a Management Team, a Transition Social Worker, Community Occupational Therapists and a Behaviour Support Therapist. 

 

What does CWD provide?

The Children with Disabilities Team (CWD) service will work with children and young people (aged between 0-18) who are defined as having a disability if they have impaired abilities in more than one of the following areas: learning, communication, sensory or physical.

 

  • We work with children and young people at threshold three and above of Tameside threshold.
 
  • The Children with Disabilities Team (CWD) service will work with those families where it is identified that support services are required that cannot be provided at a universal / enhanced local offer support.
 
  • Should a child be assessed as requiring Early Help intervention in line with Tameside’s threshold at level three, they will be allocated an Children with Disabilities Team (CWD) Intervention Worker to identify a plan of support.
 
  • Should a child be assessed as requiring intervention in line with Tameside’s threshold at level four, they will be allocated a Social Worker to identify a plan of statutory intervention and support.
 
  • The Children with Disabilities Team (CWD) service provide a through care service with those children and young people with disabilities and their siblings who are subject to care proceedings, those who may require permanent placements away from their birth families and those who are looked after by agreement with their families. Where the Children with Disabilities Team (CWD) Social service have progressed sibling groups through proceedings, discussion will take place with the Looked after Childrens Team post proceedings to transfer non-disabled siblings where appropriate.
  

Short Break Provision

As part of the overall support for children with additional needs, the provision of short breaks may form part of the support plan.
 Some short breaks form part of a continuum of different services, which support disabled children, young people and their families. Short breaks are provided to give:
 
  • Children and young people enjoyable experiences away from their primary carers, thereby contributing to their personal and social development and reducing social isolation
  • Parents, carers and families receive a necessary and valuable break from their caring responsibilities and to enable them to maintain and improve the quality of care they provide.
 
 

 Targeted complex needs short breaks

These short breaks are accessed through a social care / early help assessment and are for children and families where there is a high level of need and complexity and who require specialist provision to enable them to access short breaks and other services.  Families will usually be in receipt DLA and have an EHC plan. These short breaks can consist of a package of support that may include overnight short breaks, direct payment/individual budget, direct support in the family home and outreach support. A Social worker or Intervention worker will oversee the higher level packages and they will always be subject to an assessment of need and approval at the Children with Disabilities Team (CWD) resource panel
 

Community based short breaks

These short breaks are accessed through a social care / early help assessment and are for children and families where there is a high level of need and complexity and who require specialist provision to enable them to access short breaks and other services.  Families will usually be in receipt DLA and have an EHC plan. These short breaks may include sessions at specific clubs in locality areas across Tameside. They may also consist of a direct payment/individual budget. A Social worker or Intervention worker will oversee the higher level packages and they will always be subject to an assessment of need and approval at the Children with Disabilities Team (CWD) resource panel.
 

Enhanced local offer

These short breaks are for those children with disabilities, who are not able to access universal activities and are at risk of social exclusion.
 
These breaks can be accessed by contacting the providers directly and are not subject to a formal assessment of need. They are community based short breaks and the majority are managed and led by parent volunteers or commissioned by Tameside to deliver short breaks for children with SEND needs.
 
 
Children with Disabilities Team (CWD)’s priorities for disabled children and their families:
  • Continually improve access to services for disabled children and young people and their families
  • Ensure children and young people have access to services in their local area
  • Services to be of a high quality with regard to safety, disability knowledge and staff training
  • Demonstrate commitment towards equality of opportunity for disabled children and young people.

 

Transition

We are actively working with adult services to ensure a smooth transition process for Children with additional needs in regards to all aspects of their lives, including access to short breaks.
 
In commencing preparation for adulthood with our children and the transition worker from aged 14, adult services are then supported to  clearly understand the needs of the young person and project what services may be required for the future. This will assist adult services in their ability to continue to meet the presenting needs of the young adults within the community.

 

Who can use the Children with Disabilities Team (CWD) Service?

Jubilee Gardens

Jubilee Gardens is a Hub for Children with Disabilities and their families to access advice, information or support and a friendly and supportive environment where families can come together to access activities for the whole family to enjoy. It is a community building where we hope you will feel welcome and able to use the services and facilities. We have a Sensory room and Conference room which both families and professionals can book directly through contacting Jubilee Gardens. We have a large garden with a wheelchair accessible roundabout, both of which are additionally accessed through our support groups and the short break session that they facilitate. What facilities are there?

If you are a child with a disability we want you and your family to make full use of the facilities. If you would like to use any of the centre's rooms, please ask the staff for more details. They'll be happy to help.

 

Sensory Room

The Sensory room is a magical place filled with special lights, soft soothing music, a bubble tube and a variety of textures, mirrors and sounds. The floors and walls are cushioned and there are lights and sounds that surround you.

A Sensory room is a place of relaxation, pleasure and pure enjoyment. Anyone who enters can decide to explore and enjoy the different sensory experiences in their own way and in their own time.
 

Family room/play room:

Families can use this room to meet other families and children with disabilities enabling the whole family to relax and enjoy the facilities. The rooms are also used by support groups who use the building to provide a range of activities / short breaks for families. There is also an accessible kitchen, disabled toilets and a shower room with changing facilities available.

Conference Room:

The large conference room can be booked for support group meetings or professional meetings. There are facilities to make coffee and tea and to prepare buffets. Equipment such as WIFI, projector and flip chart are available.
Additionally the conference room is large enough to be utilised for activities, youth clubs and play sessions.

Local Offer

The Children with Disabilities Team (CWD) team work in partnership and provide grants to support the local offer.
Children with Disabilities Team (CWD) children’s services local offer is coproduced with the following support groups and agencies.
 
 
OkeOKE is a parent run support group for families living in the Tameside area who have a child/young person with special needs/disability. We're a one-stop shop for families to get help, advice and support. We also offer families access to a range of activities and events throughout the year for the whole family
 
 
T21T21 a parent led group committed to supporting parents/carers of children and young people with Down's Syndrome (Trisomy 21). We aim to provide information, advice and a friendly face to parents, carers, families and friends of people who have Down Syndrome in the Tameside and East Manchester area.
 

TASCATASCA a parent run support group for families with children 0-18 years living in the Tameside area with Autism, communication & social difficulties. We organise activities throughout the year for the whole family
 
 
 
FACT(FACT)  a parent run support group, set up to provide essential support, help and information to families of children who have been diagnosed with autism and related conditions i.e. ADHD, OCD, Pica, SPD and more complex learning disabilities, providing activities that benefit Autistic Children 5-18 years and their families.
 
 
HOPEHOPE a parent run support group for parents/carers of children and young people living with Epilepsy. We give families the opportunity to meet up, share and gain knowledge and influence service provision. We also organise activities and events throughout the year.

We are open Monday to Wednesday 8.30am - 5.00pm | Thursday 8.30am - 4.30pm | Friday 8.30am - 4.00pm

The centre is also open Saturday and Sunday for the support groups who provide short breaks for families.
