Introduction
The Children Act 1989 requires all local authorities to maintain a register of Children with Disabilities living in their area.
In Tameside the register is called the Children with Additional Needs Family Information Network Database in Tameside (Can Find It - for short!).
A group of people including parents of children with disabilities, carers, young people with disabilities, people from voluntary agencies, the Health Authorities, the Education Department, and from Social Care and Health met to develop and set up Can Find It.
The leaflets, forms and information packs are the work of these groups.
Leaflets about this service are available in the following community languages: Urdu, Gujerati, Bangla, Hindi, Farsi, Portuguese and Traditional Chinese.
What is Can Find It?
It is a new way of giving parents and carers of children and young people with additional needs free information.
It will also provide a more accurate picture of children and young people with additional needs in Tameside for the agencies to plan their services better in the future.
Help for Parents
When you join the Can Find It database we will send you a Can Find It pack, which will explain what support is available locally and nationally, who its for and how and where you go to find out more.
You will also receive copies of an Information Passport, which will have all the key information about your child. This will mean you won't have to answer the same questions over again.
Help for Child/Young Person
Your child will receive an It's Mine book about their needs which they can share with people involved in their care.
Who can be included?
Any child or young person living in Tameside and Glossop under the age of 25 who may need individual support with ordinary living. This may include needs relating to: learning, communication, behavioural, emotional, social development; hearing, sight, physical ability or significant medical conditions.
Who runs this?
Can Find It is a service run by Tameside and Glossop Primary Care and Acute (hospital) Trusts, Pennine Care Trust (mental health) and Tameside MBC Services to Children and Young People.
Who will keep the information?
This will be held on computer and will be referred to as the Children with Additional Needs Family Information Database in Tameside. It will be shared with professionals who work with your child, but only if you give your consent. This database is not the same as any other register on which your child may be included.
