You are welcome to apply for a proxy application form if you are unable to go to your Polling Station on election day.
A proxy is someone who will go to the polling station and vote with your authority on your behalf.
Complete the Vote by Proxy Application Form 68.4 KB 
if you wish to apply for a proxy application form.
The deadline for receipt of proxy applications is 6 working days before an election.
Alternatively you can pick up an application form at any of our Customer Services Offices or use the phone number below to request a form.
