What are the services?

Priority is given to NCHT tenants and is determined by the level of need, but we are also able to support a small number of people living in other tenures.

- People aged 18 - 65 years of age and currently on a Probation Order.
- West Pennine Housing Association provides support to people aged between 16 and 25 who are young parents or are pregnant and are currently living in or applying for rented accommodation in the Tameside area. The service specialises in teenage parents 16 -17 years old.
How does someone apply for support?
Referrals can be made by a wide range of organisations such as housing staff, social services, health and probation staff. People can also refer themselves for support.
Referral forms can be obtained by contacting the appropriate telephone numbers listed below.
What happens then?
Once a referral is received the support worker will make contact to arrange an initial interview in order to carry out an assessment of the persons needs and to provide more information about the service that can be delivered. This involves asking questions to help the support worker understand and assess the persons needs and will be conducted in an appropriate setting in a non-judgmental and confidential way.
A risk assessment will be carried out on all referrals and information from this will determine how support will be delivered. Some referrals may not be accepted if the risks are too high and each case will be considered individually.
Once the initial interview and risk assessment have been completed the person and the referring agency will be contacted within seven working days to inform them of what support (if any) can be provided and the next steps.
How long does the support last?
This depends on people's individual needs but support can be given for up to two years.
Regular reviews are held between the service user and the support worker in order to assess whether support is still required and to discuss progress towards agreed goals.
Withdrawal of support is planned and negotiated with the service user.
Who pays for the service?
The services are funded by a Supporting People grant from Tameside Council. This means that people who use the service do not have to pay anything.
