Checklist
Before you bring your forms into the Licensing Office, please complete this checklist to ensure that you have everything you need to apply:
- Provided DSA Assessment Certificate to Licensing Office
- Read through all documentation including Covering Letter, Topographical Test Notes and Notes of Application
- Completed Application Forms - Application Form, Equalities Questionnaire and Statutory Declaration duly signed and stamped by a Solicitor or Commissioner for Oaths
- At time of submitting application, you will need to bring -
- DVLA Drivers Licence - (if new photo card licence, please bring both parts of the licence i.e. the paper counterfeit and the I.D. Card)
- Recent passport type colour photograph,
- Proof of Address - Passport/Birth Certificate (UK birth certificate required for CRB form).
- National Insurance Number.
- Place of birth.
- Address history for where you have lived for the past 5 years.
- Provide details of all names that you have been known by.
- Fee - payment can be made at the licensing office by cheque, credit card or debit card. If you wish to pay by cash, a payment card can be obtained from the licensing office for payment at any Post Office or authorised pay point.
Note: The Criminal Records (CRB) application form will be given to you at the time of your appointment for completion.
