Governing Principles of Joint Working
Vision
Improving the health and well being of older people by providing better quality services is dependent on commitment to joint working across the social and health economy.
Tameside Older Peoples Partnership (TOPP) illustrates this commitment through its vision statement:
"To ensure the maximum quality of life and independence of vulnerable Older People by working in partnership across agencies and by using a whole systems approach."
The underlying objectives of this vision are:
- Ensure that people can remain at home during illness or following injury unless they need hospital care for clinical reasons.
- Ensure that people can be discharged from hospital safely as soon as they are fit for discharge.
- Ensure people achieve their maximum social and physical functioning following illness or injury.
- Ensure that people with long-term disabilities receive care that gives them maximum independence with safety.
- Ensure informal carers are able to provide care for as long as it is agreed is appropriate.
- Ensure equitable and easy access to services across agencies.
