We may be able to support people with physical and sensory disabilities as well as those needing long term care. You must be between eighteen and sixty five and live in Tameside.

To find out whether we are able to offer you services, we will ask you questions about your situation. We call this a Contact Assessment. Once we have gathered this information we must apply guidelines, called Eligibility Criteria, to see if you are eligible for services from us. The Government has set these eligibility criteria to ensure fair access to care services across the country. All Adult Services departments in England and Wales must apply these Criteria to help them to prioritise and make sure resources go to those in greatest need.

We currently support people with the following:

  • A Physical Disability
  • A Sensory Impairment
  • Someone who needs palliative care
  • Someone who has Acquired Brain Injury
  • Someone who has a long term illness (Chronically Sick)
  • Someone who has HIV and Aids

As well as the above we can support people with finding appropriate accommodation (this is not financial support) and the Eligibility Criteria would need to be met.

How can I get this service?

The first step is to contact Adult Services. You will need to explain what difficulties you are having. If you prefer, you can ask a friend or relative to make contact on your behalf.

If we cannot offer a service, we can give information and advice about other organisations that may be able to help.

What happens next?

If you are eligible for services, a member of the Younger Adults Assessment Team will visit and work with you on a more detailed assessment. This will cover all aspects of your daily living including your ability to access leisure/learning activities within the community.

The assessment may take an hour or more, depending on the problems that need to be discussed. If you wish, you can ask a relative, friend or carer to be with you when we visit.

Following the assessment, a care plan will be drawn up with yourself and any other professional or voluntary agencies to identify services that will be able to provide support to maximise your independency.

What happens once my care plan is in place?

We will review your package about 6 weeks from when your care starts. This is to ensure you are happy with the level of care you are receiving.

We also work in partnership with the Occupational Therapy and Moving and Handling Team , Services to the Deaf and the Visual Impairment Team.
