Section 1

Use this section to give us all your child’s personal details

• Make sure you fill the form in using CAPITAL LETTERS and black ink.
• If you have moved house within the last 3 years please tell us your previous address.
• Don’t forget to tell us your details and how we can contact you just in case we need to.
• Make sure that everyone with parental responsibility for the child knows about this form as we can only accept one application per child.

Section 2

Use this section to give us all the details we need about the schools you want to apply for

• Please put the schools you are applying for in the order that you prefer.
• The school you rank as Number 1 will be the one you want most of all.
• If you want to, you can also use this section to tell us why you want these schools.
• Don’t forget to tell us if there are any siblings who still be attending the school in September 2010.

Section 3

Use this section to give us any additional information about this application

• If your child is baptised Roman Catholic you should tick the box in this section and tell us which parish your child lives in.
• It is important to tick the relevant box if your child has a statement of Special Educational Needs or is in the care of the Local Authority.

Section 4

• If you have documentary supporting evidence which tells us about any special circumstances, please include these on separate sheets and attach the information to the application form.
• If you have completed a supplementary form or have any additional information required by voluntary aided schools, please include these with your application form.
• You should tick the box in this section if you have included any additional information.

Please remember to read the declaration and sign the form.

Finally When you give your form in remember to include your 2 proofs of address. You can find out what we will accept on the back of the application form.
