Information and advice for adults leaving Tameside General Hospital

The Transfer Service provides a one-stop, single point of contact for adults leaving hospital. The service is based at Tameside General Hospital Link to External Website and includes Social Workers, Nurses, Assessors, Customer Care Officers, Community Care Workers and administrative staff.

The Transfer Lady in hospital bedService can become involved in various ways. In Tameside Hospital, a member of the Transfer Team visits wards each day to identify patients who would benefit from our support. In other hospitals, your named nurse will be able to refer you to the Transfer Service by telephone or fax. You or your carer can also make contact with the Transfer Team direct.

By carer, we mean someone who looks after a friend, relative or neighbour who cannot manage at home without help because of their age, physical or other disability

The Transfer Service can complete an initial assessment involving yourself, your carers and the ward staff. We can provide basic advice and information on simple services that may be useful to support you back at home.

Where your discharge needs are more complex, we will arrange a further Assessment undertaken by a Social Worker, Nurse or Assessor. You will be given a copy of your Assessment.

Depending on your Assessment, various arrangements can be made:

  • Rehabilitation, either at home or in the local rehabilitation centre.
  • Social care support by a home care package or equipment, eg. Pendant alarm.
  • Where your needs are too great for you to be safely supported at home, we will discuss the provision of 24 hour care in a Residential or Nursing home.
  • Informing community health services of your discharge.

If you are not eligible for direct services from Adult Services, we will explain why. If appropriate, we will give you information and advice on other organisations which
may be able to support you after discharge from hospital.

What happens next?

After discussion, the Social Worker, Nurse or Assessor will talk with you about what actual support you need and will arrange those services necessary for your hospital
discharge.

Details about any services we arrange for you will be written down on a Care Plan. You will be given a copy of this.

You may have to make a contribution towards the cost of some services but our team member will discuss this with you.

What if you need residential or nursing care?

If you are assessed as needing residential or nursing care, the Social Worker or Nurse will:

  • Arrange a meeting with you and any others involved in your care.
  • Ensure your needs are assessed against the Continuing Health Care Link to External Website criteria
  • Give you, your relatives and carers details of local care homes Link to External Website
  • If you are well enough arrange an introductory visit to a suitable home
  • Advise on the charging system for residential and nursing care and the financial help available from Adult Services.

What happens when you go home?

  • We will ensure you have your Care Plan which contains the details of any services you will be receiving and of the companies providing your care.
  • Either ourselves or one of our community teams will be responsible for monitoring and reviewing any services you will be receiving such as home care, meals on wheels, residential or nursing care.
  • We will make sure you have the contact details of the appropriate team. Any queries after discharge should be directed to them.

Patient Advice and Liaison Service (PALS)

This patient advice and liaison service Link to External Website can help you to sort out problems related to your hospital care including giving you information and advice on making a formal complaint, telling you about independent advocacy services and directing you to places where further information and help is available.

Health Information Centre

Is managed by Tameside Libraries Service and is based in Tameside Hospital.  The centre provides health information to patients, their carers and family members whether an inpatient, outpatient or patient of a GP.  If you or your carer need any information on health, social care issues or local council services then the Health Information Centre Link to External Website can help you.

Complaints, suggestions and compliments

Feedback provides us with valuable information to improve our service to you. If you would like to make a complaint, suggestion or compliment, please contact your Social Worker or the manager of that service. If you have made a complaint and are unhappy with the response, you can contact the Complaints Manager of Tameside Adult Services.

If you are being abused or suspect that somebody you know is being abused in any way, please do not ignore it.  You do not have to give your name and any information you give will be treated confidentially. Contact the Safeguarding Adults Team.
