Sometimes after a spell in hospital or illness, people lose confidence and are worried about their ability to cope at home.

Reablement is a short-term, non-chargeable service provided in your own home, giving you the opportunity to regain lost skills. We will work with you for up to 6 weeks to find new ways to do things independently, which may be through aids, equipment, or technology.

 

The Process

Following a visit, a Reablement Support Worker will work with you to re-gain lost skills by focusing on your strengths and setting achievable goals.

The level of support is different for everyone and is based on your own skills and abilities. We will work closely with you and other healthcare professionals to support you, always listening to you and your Carers.

Your goals will be recorded as part of your short-term support plan and your achievements can be monitored as you work towards your goals.

Support Workers will monitor the level of support you need daily, and your short-term package of support may increase or decrease. Reablement may be provided for a maximum of 6 weeks and a review can be triggered at any time.
 

After Reablement

At the end of Reablement, if your goals for independence have been met, you will be discharged from the service.

If you need longer-term care and support, a social worker or assessor will review your situation with yourself and if needed, any family members or advocates.
