Tameside D Scheme for Cared for Children and Care Leavers

Roles and Responsibilities of Health and Social Care Professionals

1. The D Scheme is used when a Cared for Child or Care Leaver is in need of a dentist and there have been failed attempts to get an NHS Dentist for them.  (If this is an emergency situation, the GM Dental Pathway to be followed and you will need to make contact with Joanne Brown Social Worker at joanne.brown@tameside.gov.uk) or dial 111 if this is out of hours
2. Scan the following QR Code on your Smart or Android Phone/Device 
3. This QR Code will direct you to the D Scheme website where you can obtain the details of the nearest NHS Dentist that have signed up to the D Scheme
4. 
5. Make contact with the NHS Dentist and arrange an appointment
6.  The NHS Dentist receptionist to inform the caller that they need consent to check with the NHS Cared for Children Team in order to check the child’s cared for status / care experienced status.  The NHS Dentist to email  caredforchildren@tgh.nhs.uk  whom will respond within 48 hours. Once verified, an appointment will be made and any queries responded to
7.  All attempts to be made for the Cared for Child / Care Leaver to either be accompanied or encouraged to attend by either their foster carer; residential care staff; personal assistant; health professional or social worker
8.  The participating D Scheme Dentist will have 3 monthly support meetings held by the Cared for Children Team via Microsoft Teams in order for any issues to be rectified.
