Please note, Healthy Start application forms no longer need PART B signing and stamping before sending off, meaning families can send the application form off without PART B being completed.
 
You can call the National Healthy Start Scheme helpline if you are struggling to print off or complete your application, please call 0345 607 6823. The call handler will complete the application form with the you over the phone and they will then print your completed application form out and send it to you (along with a freepost envelope) in the post awaiting your signature. You will then need to send your signed application form off in the post using the freepost envelope that was sent out with your application form.
 
Alternatively where possible, applications still need to be completed on paper, signed by the applicant and posted to the Healthy Start issuing unit. If you think you are eligible for the Healthy Start scheme please click here to download the application form and please complete PART A (sections 1-7).
If you are struggling to download and print the application form off, please call Tameside Customer Services on 0161 342 3494 and 0161 342 2284 or email welfare@tameside.gov.uk who will be happy to print you an application form off and send it to you in the post for you to complete. Once you receive your application form you will then need to complete it and send it off in the post to:

Having access to good nutrition is key to supporting the very best start in life for little ones and Healthy Start vouchers can help families give their little ones just that.

The Healthy Start Vouchers can be used at local shops support families to buy milk and fresh or frozen fruit and vegetables. The vouchers can also be exchanged for free Healthy Start vitamins for pregnant women, breastfeeding women and children aged from six months to four years.

You could qualify to receive Healthy Start Vouchers, so don’t miss out on receiving vouchers worth up to £6.20 a week per child to spend on fruit, veg, milk and vitamins.

You qualify for Healthy Start vouchers if you’re at least 10 weeks pregnant or have a child under four years old and you and your family get:
  • Income support
  • Income-based Jobseekers Allowance
  • Income-related Employment and Support Allowance
  • Child Tax Credit (with a family income of £16,190 or less per year)
  • Universal Credit (with a family take home pay of £408 or less per month)

If you think you qualify, all you need to do is complete your application form, these are easy to get hold of and can be found online or your key worker may already have one to hand.

If you have a key worker who can support you in completing your Healthy Start application form and you think you could do with a little support in completing it, ask for their support.

Pop it in the post using the FREEPOST envelope.
