Eligibility Criteria

If you are aged 18 and over and live in Tameside, you can request an assessment for care and support. Carers can also request a Carers Assessment to access support.
 
Eligibility for help is based on looking at how a person's needs affect their ability to achieve their goals and how this impacts on their wellbeing.

When considering whether an adult has eligible care and support needs, we must consider whether:
  1. You have needs arising from or related to a physical or mental impairment or illness.
  2. As a result of your physical or mental impairment you are unable to achieve two or more of the tasks below:
  • managing and maintaining nutrition.
  • maintaining personal hygiene.
  • managing toilet need.
  • being appropriately clothed.
  • being able to make use of the adult’s home safely.
  • maintaining a habitable home environment.
  • developing and maintaining family or other personal relationships.
  • accessing and engaging in work, training, education, or volunteering.
  • making use of necessary facilities or services in the local community including public transport, and recreational facilities or services; and
  • carrying out any caring responsibilities the adult has for a child.
  1. Due to not being able to carry out these tasks there is likely to be a significant impact on your wellbeing.

If you are Eligible

If you are eligible to receive care and support, we will contact you to arrange a Support Plan to be created.

Information on what the Support Plan is, how it is arranged, monitored and any costs associated.

If you are not Eligible

If you are not eligible to receive care and support through Adults Social Care, we may be able to signpost you to other local services who can help to maintain your health, wellbeing, and independence to live well at home.
 
To access these services an assessment does not need to be completed and there are no eligibility criteria to meet.
